Pre-Award Forms	Description
Proposal Approval Form	Form should be completed, signed through the Vice Presidential level and submitted with the complete proposal to the Sponsored Programs Office (SPO).
Preliminary Proposal Approval Form	Form should be completed, signed through the Vice Presidential level and submitted to the SPO with a preliminary proposal, letter of inquiry, white paper or pre-application.
Cost Sharing Identification and Approval Form	Form should be completed, signed and submitted with the proposal to describe the specific expenses proposed as cash and/or in-kind match or cost-share (if applicable).
Human Subjects Review Form	Principal Investigators/Project Directors whose research involves the use of human subjects must complete this form prior to submission of proposals
Use of Laboratory Animals Form	Principal Investigators/Project Directors whose research involves the use of vertebrate animals must complete this form prior to submission of proposals.
Use of Hazardous Materials Form	Principal Investigators/Project Directors whose research involves the use of biohazardous agents, potentially toxic or hazardous chemicals or drugs must complete this form prior to submission of proposals.
Post-Award Forms	Description
Budget Request Form	Prepare in advance and submit this form at the Account Create meeting to establish a new or continuation sponsored grant account.
Cost Share Budget Request Form	Prepare and submit this form at the Account Create meeting for a new or continuation grant that includes cash or in-kind match or cost-share.
Budget Revision Form	Form should be completed, signed through the Vice Presidential level and submitted to the Sponsored Programs Office with justification to request a modification to the grant budget. (Other conditions may apply, such as prior sponsor approval.)
