Financial Conflict of Interest Disclosure

Disclosure of potential conflicts of interest is required for certain employee-third party financial relationships and certain circumstances involving an employee’s intellectual property. See University Policy Number 200.21: Policy on Conflict of Interest, and the Sponsored Programs Office Policy on Intellectual Property.

Proposal Approval Form

Form should be completed, signed through the Vice Presidential level and submitted with the complete proposal to the Sponsored Programs Office (SPO).

Form should be completed, signed and submitted with the proposal to describe the specific expenses proposed as cash and/or in-kind match or cost-share (if applicable).

Human Subjects Review Form 

Principal Investigators/Project Directors whose research involves the use of human subjects must complete this form prior to submission of proposals.

Prepare in advance and submit this form at the Account Create meeting to establish a new or continuation sponsored grant account.

Form should be completed, signed through the Vice Presidential level and submitted to the Sponsored Programs Office with justification to request a modification to the grant budget. (Other conditions may apply, such as prior sponsor approval.)
